For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Ceftriaxone can precipitate with calcium in LR; neonatal coadministration is contraindicated. In others, do not mix or Y-site co-infuse; risk of fatal pulmonary/renal precipitates.
Mechanism of action
Physicochemical precipitation: ceftriaxone chelates Ca2+ → insoluble ceftriaxone–calcium salts → emboli/organ deposition.
Clinical management
Neonates ≤28 days: contraindicated with any IV calcium-containing solution. >28 days: avoid mixing or Y-site coadministration; do not reconstitute/dilute with LR; infuse sequentially with thorough line flush using compatible fluid (e.g., NS).
