Summary

Oral fecal microbiota products contain live bacterial spores meant to re-establish the gut flora after Clostridioides difficile infection. Antibacterial agents active against these organisms can suppress engraftment and lower efficacy. Clindamycin has broad anaerobic activity and is itself a major risk factor for C. difficile.

What this interaction means in plain words

Clindamycin and Microbiota can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Antibacterial activity against the administered live bacterial spores and the resulting flora impairs engraftment and colonization.

What to do

Complete any antibiotic course before starting the microbiota product, observing the interval in the product labeling. Avoid starting clindamycin during or soon after microbiota therapy unless clearly necessary.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
