Summary

Protriptyline is a strongly anticholinergic tricyclic antidepressant, which adds to the antimuscarinic effects of darifenacin. Darifenacin's CYP2D6 inhibition may also raise protriptyline levels, increasing the risk of cardiac and anticholinergic effects.

What this interaction means in plain words

Darifenacin and Protriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive muscarinic antagonism plus possible CYP2D6 inhibition by darifenacin.

What to do

Monitor for anticholinergic toxicity, tachycardia, and urinary retention, and consider an ECG or lower protriptyline dose in older or cardiac patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
