Summary

Aminoglycosides such as tobramycin can impair neuromuscular transmission and may potentiate botulinum toxin. This could cause excessive muscle weakness, including swallowing or breathing difficulty, particularly with systemic absorption.

What this interaction means in plain words

Dexamethasone-tobramycin and Incobotulinumtoxina can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive impairment of neuromuscular transmission, with aminoglycosides reducing presynaptic acetylcholine release on top of the toxin's blockade.

What to do

Avoid starting systemic aminoglycoside therapy around the time of botulinum toxin injection when alternatives exist. If both are required, monitor for generalized weakness, dysphagia and respiratory compromise.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
