Summary

Corticosteroids and thiazide diuretics both promote potassium loss, so using them together can cause additive hypokalemia. Low potassium raises the risk of arrhythmias and muscle weakness. Topical dexamethasone-tobramycin produces little systemic effect.

What this interaction means in plain words

Dexamethasone-tobramycin and Methyclothiazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Additive renal potassium wasting through mineralocorticoid activity of the corticosteroid and distal tubular sodium delivery from the thiazide.

What to do

Monitor serum potassium if systemic exposure to dexamethasone is significant, and supplement potassium if needed.
