Summary
Oral neomycin is poorly absorbed, but absorption can increase with damaged gut mucosa or renal impairment. Combined with tobramycin, which is also oto- and nephrotoxic, this can add to kidney and inner-ear injury when tobramycin is given systemically. With topical or ophthalmic tobramycin, systemic exposure is minimal and the risk is low.
What this interaction means in plain words
Dexamethasone-tobramycin and Neomycin po can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxic and ototoxic effects of two aminoglycosides.
What to do
Avoid combining systemic aminoglycosides with oral neomycin where possible. If used, monitor renal function, aminoglycoside levels and hearing, particularly in renal impairment.
