Summary
Aminoglycosides such as tobramycin can potentiate neuromuscular blockade from nondepolarizing agents, prolonging paralysis and risking respiratory depression or apnea. This applies mainly when tobramycin is given systemically. Rapacuronium has been withdrawn from most markets.
What this interaction means in plain words
Dexamethasone-tobramycin and Rapacuronium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Aminoglycosides reduce presynaptic acetylcholine release and postsynaptic sensitivity, adding to nondepolarizing neuromuscular blockade.
What to do
Avoid or monitor neuromuscular function with a nerve stimulator and ensure ventilatory support is available. Reduce the muscle relaxant dose as needed.
What to ask your pharmacist
