Summary
Dichlorphenamide, a carbonic anhydrase inhibitor, causes hyperchloremic metabolic acidosis. Nitroprusside infusion, especially at high doses or prolonged use, can cause lactic acidosis through cyanide accumulation, so acid-base disturbance may be compounded.
What this interaction means in plain words
Dichlorphenamide and Sodium nitroprusside can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dichlorphenamide belongs to Miscellaneous; Sodium nitroprusside belongs to Cardiovascular.
Why it happens
Additive acidosis: renal bicarbonate loss from carbonic anhydrase inhibition plus cyanide-related impaired cellular respiration from nitroprusside.
What to do
Monitor blood gases, bicarbonate and lactate during nitroprusside infusion in patients on dichlorphenamide; keep nitroprusside dose and duration to a minimum and treat unexplained acidosis as possible cyanide toxicity.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
