Summary

Fexinidazole may inhibit CYP3A4 and raise diltiazem concentrations, increasing the risk of bradycardia, AV block and hypotension. Fexinidazole itself also prolongs the QT interval.

What this interaction means in plain words

Diltiazem and Fexinidazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Fexinidazole inhibits CYP3A4, reducing diltiazem metabolism.

What to do

Monitor heart rate, blood pressure and ECG during the short fexinidazole course. Reduce the diltiazem dose if bradycardia or hypotension develops.

What to ask your pharmacist
