Summary
Metipranolol is a nonselective beta blocker given as eye drops, but some drug is absorbed systemically. With diltiazem, which slows sinus and AV node conduction, there is a possibility of additive bradycardia, AV block or hypotension, especially in susceptible patients.
What this interaction means in plain words
Diltiazem and Metipranolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Additive negative chronotropic and dromotropic effects from beta blockade and calcium channel blockade.
What to do
Monitor heart rate and ECG in patients with conduction disease or on high-dose diltiazem when starting metipranolol drops. Use punctal occlusion to reduce systemic absorption.
What to ask your pharmacist
