Fexinidazole can prolong the QT interval and its labeling advises against co-use with other QT-prolonging drugs. Donepezil adds torsades risk, so the combination may provoke a serious ventricular arrhythmia.
What this interaction means in plain words
Donepezil and Fexinidazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive prolongation of cardiac repolarization through potassium channel effects.
What to do
Avoid the combination, and consider withholding donepezil during the short fexinidazole course. If this is not possible, obtain an ECG and correct electrolytes before and during treatment.
What to ask your pharmacist
