Summary
Donepezil increases cholinergic tone and may worsen or provoke extrapyramidal and other adverse effects, while pimozide's dopamine blockade and anticholinergic properties may blunt cholinergic benefit. Donepezil can also cause bradycardia and has been linked to QT prolongation and torsades de pointes, which adds to the arrhythmia risk of pimozide.
What this interaction means in plain words
Donepezil transdermal and Pimozide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Pharmacodynamic antagonism of cholinergic and dopaminergic effects, plus additive QT-prolonging and bradycardic effects.
What to do
Obtain a baseline ECG and monitor QTc, heart rate and electrolytes if the combination is needed. Watch for worsening movement disorders, and consider an alternative to one agent if QTc lengthens.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
