Summary

Trastuzumab can cause left ventricular dysfunction and heart failure, and the risk is greatest when it is given with or soon after anthracyclines. Liposomal doxorubicin is less cardiotoxic than conventional doxorubicin but still adds cardiac risk.

What this interaction means in plain words

Doxorubicin liposomal and Trastuzumab can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

Additive cardiotoxicity: anthracycline myocyte injury combined with impaired HER2-dependent cardiomyocyte repair from trastuzumab.

What to do

Avoid the combination if possible. If it is used, assess left ventricular ejection fraction before and at regular intervals during treatment, and stop trastuzumab for significant decline.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
