Summary

Droperidol is a potent dopamine D2 antagonist, and ropinirole is a D2 agonist, so each reduces the other's effect. Droperidol may worsen parkinsonism and blunt ropinirole's benefit. Droperidol also prolongs the QT interval.

What this interaction means in plain words

Droperidol and Ropinirole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism at dopamine D2 receptors.

What to do

Avoid droperidol in patients with Parkinson disease and use an alternative antiemetic such as domperidone where available or ondansetron with ECG awareness. If needed, monitor motor function closely.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
