Summary
Tenofovir is eliminated by the kidney and is nephrotoxic, and NSAIDs can reduce renal perfusion. Concurrent use, particularly with high-dose or prolonged NSAID therapy, increases the risk of acute kidney injury and tenofovir-related tubular toxicity. Cobicistat also raises serum creatinine by blocking tubular secretion, which can mask changes.
What this interaction means in plain words
Elvitegravir / cobicistat / emtricitabine / tenofovir df and Fenoprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxicity from NSAID-mediated reduction in renal blood flow and tenofovir proximal tubular injury, with competition for renal elimination.
What to do
Prefer a non-NSAID analgesic such as acetaminophen. If an NSAID is necessary, use the lowest dose briefly and monitor renal function, including phosphate and urine protein.
What to ask your pharmacist
