Summary

Heparin can suppress aldosterone secretion and raise serum potassium. With enalapril, which also lowers aldosterone, hyperkalemia is more likely, especially in patients with renal impairment or diabetes.

What this interaction means in plain words

Enalapril and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Enalapril belongs to Cardiovascular; Heparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive aldosterone suppression from heparin and ACE inhibition, resulting in potassium retention.

What to do

Monitor serum potassium and renal function if heparin is given for more than a few days. Treat hyperkalemia and review other potassium-raising drugs if it occurs.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
