Summary
Both drugs stimulate adrenergic receptors, so using them together can add to tachycardia, palpitations, tremor, arrhythmia risk and blood pressure changes. Both can also lower serum potassium through beta-2 stimulation, which increases arrhythmia risk, particularly in patients with asthma who are hypoxic or taking other potassium-lowering drugs.
What this interaction means in plain words
Epinephrine and Metaproterenol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive beta-adrenergic agonism causing cardiovascular stimulation and intracellular potassium shift.
What to do
Monitor heart rate, rhythm and blood pressure, and check serum potassium when high doses or repeated dosing of both are given. Avoid unnecessary concurrent use of multiple sympathomimetic bronchodilators.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

How serious is the interaction between Epinephrine and Metaproterenol?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor heart rate, rhythm and blood pressure, and check serum potassium when high doses or repeated dosing of both are given. Avoid unnecessary concurrent use of multiple sympathomimetic bronchodilators.
