Summary
Olanzapine blocks alpha-1 adrenergic receptors, so if epinephrine is given for hypotension or shock, beta-mediated vasodilation may predominate and blood pressure may fall further. This paradoxical effect is classically described for antipsychotics with strong alpha-blocking activity.
What this interaction means in plain words
Epinephrine and Olanzapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Epinephrine belongs to Endocrine; Olanzapine belongs to Central Nervous System.
Why it happens
Alpha-1 adrenergic blockade by olanzapine reverses the pressor effect of epinephrine, leaving unopposed beta-2 vasodilation.
What to do
If a vasopressor is needed in a patient taking olanzapine, prefer norepinephrine or phenylephrine rather than epinephrine. Monitor blood pressure closely when epinephrine is required, such as in anaphylaxis.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?

Frequently asked questions
Can I take Epinephrine with Olanzapine?
MediHelp rates the interaction as major. Epinephrine and Olanzapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
How serious is the interaction between Epinephrine and Olanzapine?
MediHelp rates it major, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: If a vasopressor is needed in a patient taking olanzapine, prefer norepinephrine or phenylephrine rather than epinephrine. Monitor blood pressure closely when epinephrine is required, such as in anaphylaxis.
