Summary
Antipsychotics with alpha-adrenergic blocking activity can reverse the pressor effect of epinephrine, leaving beta-mediated vasodilation unopposed and causing hypotension and reflex tachycardia. This is relevant when epinephrine is used to treat shock or anaphylaxis in a patient taking thiothixene.
What this interaction means in plain words
Epinephrine and Thiothixene can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Alpha-1 receptor blockade by thiothixene unmasks the beta-2 vasodilatory action of epinephrine (epinephrine reversal).
What to do
Monitor blood pressure closely if epinephrine is required and be prepared to use a pure alpha agonist such as norepinephrine or phenylephrine if hypotension persists.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
