Summary

Xylometazoline is an alpha-adrenergic vasoconstrictor, and epinephrine adds further adrenergic stimulation. Together they can raise blood pressure and heart rate more than either drug alone, which matters in patients with hypertension or heart disease. Xylometazoline is applied intranasally, so the effect is mainly seen with heavy use or in susceptible patients.

What this interaction means in plain words

Epinephrine and Xylometazoline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Epinephrine belongs to Endocrine; Xylometazoline belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive alpha- and beta-adrenergic agonist effects produce cardiovascular stimulation and vasoconstriction.

What to do

Monitor blood pressure and heart rate when systemic or injected epinephrine is given to a patient using xylometazoline. Limit decongestant use in patients with cardiovascular disease.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
