Summary

Yohimbine blocks presynaptic alpha-2 receptors and increases norepinephrine release, raising blood pressure and heart rate. Together with epinephrine, hypertension, tachycardia, anxiety and arrhythmias may be amplified.

What this interaction means in plain words

Epinephrine and Yohimbine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive sympathomimetic effects from alpha-2 antagonism by yohimbine and direct adrenergic agonism by epinephrine.

What to do

Monitor blood pressure and heart rate when epinephrine is given and avoid unnecessary epinephrine in patients using yohimbine.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
