Escitalopram + Orphenadrine

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive CNS depression (sedation, dizziness, impaired psychomotor function) and a potential additive effect on QT prolongation, which may increase torsades de pointes risk in susceptible patients (e.g., baseline QT prolongation, electrolyte disturbances, structural heart disease, higher doses).

Mechanism of action

Pharmacodynamic: Orphenadrine has anticholinergic and antihistaminic sedative effects; escitalopram can contribute to CNS effects. Escitalopram prolongs QT via hERG blockade; orphenadrine has quinidine-like cardiac effects and has been associated with arrhythmias, suggesting possible QT effects.

Clinical management

Prefer alternatives in patients with significant QT risk. If coadministered: use lowest effective doses; avoid other QT‑prolonging agents and CNS depressants; obtain baseline and follow‑up ECG in patients with cardiac risk factors; correct hypokalemia/hypomagnesemia; counsel on sedation and falls; caution with driving or operating machinery.
