Summary

Alcohol taken with nitroimidazoles may cause a disulfiram-like reaction with flushing, nausea, vomiting, abdominal pain and headache. Secnidazole labeling advises avoiding alcohol during and shortly after therapy, although the reaction is less well documented than with metronidazole.

What this interaction means in plain words

Ethanol and Secnidazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Proposed inhibition of aldehyde dehydrogenase causing acetaldehyde accumulation; the mechanism is not firmly established for secnidazole.

What to do

Advise patients to avoid alcohol and ethanol- or propylene glycol-containing products during treatment and for 2 days after the last dose.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
