Summary
Fenbufen is an NSAID that can reduce renal prostaglandin synthesis, blunting the antihypertensive effect of olmesartan and diuretics. In volume-depleted, elderly, or renally impaired patients, the combination can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Fenbufen and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion and sodium excretion, which, with RAS blockade and diuretics, impairs renal function and potassium excretion.
What to do
Avoid prolonged NSAID use where possible; ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium after starting and during therapy.
What to ask your pharmacist
