Summary
NSAIDs such as fenbufen can reduce the antihypertensive effect of olmesartan and increase the risk of acute kidney injury, particularly in elderly, dehydrated or renally impaired patients. Hyperkalemia may also occur.
What this interaction means in plain words
Fenbufen and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, which antagonizes ARB effects and impairs glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium, and keep patients well hydrated. Use the shortest possible NSAID course or choose an alternative analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
