Summary

NSAIDs can reduce the antihypertensive effect of angiotensin receptor blockers and, in volume-depleted or elderly patients, may cause acute kidney injury. Hyperkalemia is also more likely.

What this interaction means in plain words

Fenoprofen and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fenoprofen belongs to Musculoskeletal; Telmisartan belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandins reduces afferent arteriolar dilation, which combined with ARB efferent dilation lowers glomerular filtration and impairs sodium and potassium handling.

What to do

Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting the NSAID. Avoid prolonged use in older or volume-depleted patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
