Summary
Fexinidazole may inhibit CYP enzymes and could raise theophylline exposure. Since theophylline has a narrow therapeutic range, even modest increases could cause tremor, tachycardia, arrhythmia or seizures. The clinical size of the effect is not well characterised.
What this interaction means in plain words
Fexinidazole and Theophylline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Fexinidazole and its metabolites inhibit CYP enzymes including CYP3A4, which may reduce theophylline clearance.
What to do
Monitor theophylline serum concentrations and signs of toxicity during fexinidazole treatment, and reduce the theophylline dose if levels rise.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
