Fluoroquinolones can cause both hypoglycemia and hyperglycemia, and the risk is highest in diabetic patients taking sulfonylureas. Severe hypoglycemia, sometimes with coma, has been reported with this class.

What this interaction means in plain words

Fleroxacin and Glibenclamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Fluoroquinolones may stimulate pancreatic insulin release by blocking beta-cell potassium channels, adding to the effect of glyburide.

What to do

Monitor blood glucose closely during the antibiotic course and warn patients about hypoglycemia symptoms. Consider a different antibiotic class in high-risk patients such as the elderly or those with renal impairment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
