For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive CNS depressant effects and therapeutic duplication of α2δ ligands, increasing risk of dizziness, somnolence, ataxia, falls, and peripheral edema; potential respiratory depression in high-risk patients or with other CNS depressants.

Mechanism of action

Pharmacodynamic: combined α2δ-1/α2δ-2 calcium channel subunit modulation enhances CNS depressant effects; no meaningful pharmacokinetic interaction (both primarily renally eliminated).

Clinical management

Avoid concurrent use; select a single gabapentinoid. If switching, cross-taper to minimize withdrawal/adverse effects. If coadministration is unavoidable, use lowest effective doses, adjust for renal impairment, and monitor for excessive sedation, respiratory depression, gait instability, and edema.
