Summary
Henbane contains hyoscyamine and scopolamine, which are antimuscarinic alkaloids that oppose the muscarinic effects of pyridostigmine. Pyridostigmine's benefit on gut, bladder and secretions may be blunted, while its nicotinic benefit at the neuromuscular junction is largely retained. Antimuscarinics can also mask early cholinergic toxicity symptoms.
What this interaction means in plain words
Henbane and Pyridostigmine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Pharmacodynamic antagonism: muscarinic receptor blockade opposes the acetylcholine accumulation produced by cholinesterase inhibition.
What to do
Avoid unnecessary use of henbane-containing products in myasthenia patients. If an antimuscarinic is needed, monitor muscle strength and watch for cholinergic crisis, as its muscarinic warning signs may be masked.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
