Summary

Heparin can suppress aldosterone secretion, which may reduce potassium excretion. With olmesartan this may lead to hyperkalemia, especially in patients with renal impairment or diabetes. The thiazide partly offsets the effect.

What this interaction means in plain words

Heparin and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Heparin belongs to Cardiovascular; Hydrochlorothiazide-olmesartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced suppression of aldosterone synthesis adds to angiotensin receptor blockade, reducing renal potassium excretion.

What to do

Monitor serum potassium if heparin is continued beyond several days, particularly in patients at risk of hyperkalemia.
