Summary

Meclofenamate is an NSAID that reduces renal prostaglandin synthesis, which can blunt the antihypertensive and diuretic effects of the combination. Together with an ARB and thiazide, NSAIDs increase the risk of acute kidney injury, especially in dehydrated or older patients, and may cause hyperkalemia.

What this interaction means in plain words

Hydrochlorothiazide-olmesartan and Meclofenamate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

NSAID inhibition of vasodilatory renal prostaglandins reduces renal perfusion and sodium excretion, which with RAS blockade and diuretic therapy impairs renal function.

What to do

Use the lowest effective NSAID dose for the shortest time and keep patients hydrated. Monitor blood pressure, serum creatinine and potassium.

What to ask your pharmacist
