Summary

Beta-2 agonists can lower serum potassium, and thiazides do so as well. This may cause clinically relevant hypokalemia, mainly with high doses or in patients with other risk factors, and may raise arrhythmia risk.

What this interaction means in plain words

Hydrochlorothiazide-olmesartan and Olodaterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Beta-2 stimulation shifts potassium into cells and thiazides increase renal potassium excretion, giving additive hypokalemia.

What to do

Check serum potassium and magnesium in patients with additional risk factors such as heart disease. Replace potassium if low.
