Summary

Osmotic bowel preparations cause fluid loss and electrolyte shifts. In patients taking a thiazide diuretic and an angiotensin receptor blocker, volume depletion and electrolyte disturbance can be amplified, raising the risk of arrhythmia, seizures and acute kidney injury.

What this interaction means in plain words

Hydrochlorothiazide-olmesartan and Sodium sulfate / ?magnesium sulfate / potassium chloride can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive fluid and electrolyte depletion from bowel-prep diarrhea plus diuretic-induced natriuresis and ARB-dependent reduction in renal perfusion autoregulation.

What to do

Ensure adequate hydration and consider holding the diuretic and olmesartan on the day of bowel preparation. Check electrolytes and renal function in high-risk patients such as the elderly or those with kidney disease.

What to ask your pharmacist
