Summary
Bowel preparations cause fluid and electrolyte losses, and with a thiazide diuretic the risk of volume depletion, hypokalemia, hyponatremia and hypomagnesemia increases. Olmesartan may add to the risk of acute kidney injury in a dehydrated patient.
What this interaction means in plain words
Hydrochlorothiazide-olmesartan and Sodium sulfate / potassium chloride / magnesium sulfate / polyethylene glycol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive fluid and electrolyte loss from osmotic diarrhea and thiazide-induced renal losses.
What to do
Consider holding the diuretic and olmesartan combination on the day of bowel preparation, and ensure adequate hydration. Check electrolytes and renal function in high-risk patients.
