Summary
Trilostane inhibits adrenal steroid synthesis including aldosterone, which reduces potassium excretion. With olmesartan it may increase hyperkalemia risk, and it adds to the natriuretic effect and blood pressure lowering of the thiazide.
What this interaction means in plain words
Hydrochlorothiazide-olmesartan and Trilostane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of 3-beta-hydroxysteroid dehydrogenase lowers aldosterone, adding to angiotensin receptor blockade and altering thiazide potassium effects.
What to do
Monitor serum potassium, sodium and blood pressure when combined, especially in renal impairment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
