Summary

Xipamide is a thiazide-like diuretic, so combining it with hydrochlorothiazide duplicates therapy and increases diuresis. This can cause hypotension, hyponatremia, hypokalemia and renal impairment, and olmesartan adds to the blood pressure and renal effects.

What this interaction means in plain words

Hydrochlorothiazide-olmesartan and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive inhibition of distal tubular sodium chloride reabsorption together with ARB-related blood pressure lowering.

What to do

Avoid duplicating thiazide-type diuretics unless intended, and monitor electrolytes, renal function and blood pressure if combined.
