Summary
Xipamide is a thiazide-like diuretic that causes potassium loss, and hydrocortisone has mineralocorticoid activity that also promotes potassium excretion. Combined use increases the risk of hypokalemia, which can provoke arrhythmias and muscle weakness.
What this interaction means in plain words
Hydrocortisone and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive renal potassium wasting from diuretic action and corticosteroid mineralocorticoid effect.
What to do
Monitor serum potassium and magnesium periodically and supplement or add a potassium-sparing agent if levels fall. Take extra care in patients on digoxin or with cardiac disease.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
