Summary

Thiazide-type diuretics can raise blood glucose and may modestly weaken the glucose-lowering effect of metformin. At the low doses of indapamide typically used for hypertension the effect is usually small. Glycaemic control may need review after the diuretic is started.

What this interaction means in plain words

Indapamide and Metformin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Indapamide belongs to Cardiovascular; Metformin belongs to Endocrine.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism: thiazide-type diuretics can impair insulin secretion and sensitivity, partly through potassium loss.

What to do

Check blood glucose or HbA1c after starting or changing indapamide and adjust antidiabetic therapy if control worsens. Keep serum potassium in the normal range.
