Summary

Combining two thiazide-type diuretics gives additive diuresis and additive loss of potassium and sodium. This raises the risk of hypokalemia, hyponatremia and volume depletion, and low potassium can trigger arrhythmias.

What this interaction means in plain words

Indapamide and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Both drugs inhibit the distal tubular sodium-chloride cotransporter, producing additive urinary electrolyte loss.

What to do

Avoid duplicate thiazide-type therapy where possible. If both are used, monitor serum potassium, sodium and renal function and correct deficits.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
