Summary

Combining an angiotensin receptor blocker with a thiazide-like diuretic gives additive blood pressure lowering, and the opposing effects on potassium usually offset one another. Volume depletion, first-dose hypotension and electrolyte disturbances (low sodium or abnormal potassium) can occur, especially in older patients or those with kidney disease.

What this interaction means in plain words

Indapamide and Telmisartan-amlodipine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Why it happens

Additive antihypertensive pharmacodynamic effects; telmisartan reduces aldosterone-mediated potassium loss while indapamide promotes renal potassium and sodium excretion.

What to do

Check blood pressure, serum potassium, sodium and creatinine after starting or changing doses. Correct volume depletion before starting.
