Summary
Intravascular iodinated contrast can cause an acute fall in renal function, and because metformin is cleared by the kidney it may accumulate. Rare cases of metformin-associated lactic acidosis have followed contrast administration, particularly in patients with impaired renal function.
What this interaction means in plain words
Ioxilan and Metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Contrast-induced acute kidney injury reduces renal metformin elimination, raising metformin levels and the risk of lactic acidosis.
What to do
Check eGFR before the procedure and withhold metformin in patients with eGFR below 30, with acute kidney injury, or receiving intra-arterial contrast. Restart metformin about 48 hours afterwards only once renal function has been re-checked and found stable.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
