Summary

Protriptyline is a tricyclic antidepressant with relatively modest sedation, but it can add to ketamine's CNS effects. Tricyclics combined with ketamine may also increase the risk of hypertension, tachycardia and arrhythmia because both potentiate sympathetic activity.

What this interaction means in plain words

Ketamine and Protriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive CNS depression plus combined sympathomimetic and anticholinergic cardiovascular effects.

What to do

Monitor heart rate, blood pressure, ECG and sedation level during ketamine use. Titrate ketamine cautiously.
