Summary

Thiazide diuretics waste potassium, and beta-2 agonists drive potassium into cells. Together they can cause clinically significant hypokalemia, which may provoke arrhythmias, especially with high-dose or repeated levosalbutamol.

What this interaction means in plain words

Levosalbutamol and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive lowering of serum potassium through renal loss (thiazide) and intracellular shift (beta-2 agonist).

What to do

Monitor serum potassium, particularly during acute asthma or COPD exacerbations or with high-dose nebulized therapy. Replace potassium as needed.

What to ask your pharmacist
