Summary
Topiramate inhibits carbonic anhydrase and can cause hyperchloremic non-anion gap metabolic acidosis. Metformin carries a risk of lactic acidosis, so the combination may worsen acid-base disturbance, and topiramate can modestly raise metformin exposure.
What this interaction means in plain words
Metformin and Topiramate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Metformin belongs to Endocrine; Topiramate belongs to Central Nervous System.
Why it happens
Additive acidosis risk from carbonic anhydrase inhibition by topiramate and metformin-associated lactate accumulation, with possible reduced renal clearance of metformin.
What to do
Check serum bicarbonate at baseline and periodically, and monitor renal function and glucose. Reduce the dose or stop one drug if persistent acidosis develops, and watch for symptoms such as malaise or hyperventilation.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
