Summary
Thiazide-like diuretics such as xipamide can impair glucose tolerance, which may reduce the glucose-lowering effect of metformin. Diuretic-induced volume depletion or reduced renal function may also raise the risk of metformin accumulation and lactic acidosis.
What this interaction means in plain words
Metformin and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Xipamide can cause hyperglycemia through potassium loss and reduced insulin secretion, and volume depletion may reduce renal metformin clearance.
What to do
Monitor blood glucose, renal function and hydration status when starting or adjusting xipamide. Adjust the antidiabetic regimen as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
