Summary

Both drugs inhibit carbonic anhydrase, so together they raise the risk of metabolic acidosis and of kidney stone formation. Urine becomes more alkaline with lower citrate, favouring calcium phosphate stones.

What this interaction means in plain words

Methazolamide and Topiramate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive carbonic anhydrase inhibition causing bicarbonate loss, alkaline urine and hypocitraturia.

What to do

Monitor serum bicarbonate at baseline and periodically, and encourage adequate hydration. Reduce the dose or stop one agent if persistent acidosis or stones develop.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
