Summary
Sacubitril inhibits neprilysin, which degrades bradykinin, and ACE inhibitors block another bradykinin-degrading pathway. Together they markedly raise the risk of angioedema, which can be life-threatening. Valsartan also adds dual renin-angiotensin blockade.
What this interaction means in plain words
Moexipril and Sacubitril / valsartan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Combined inhibition of neprilysin and ACE causes bradykinin accumulation, with added renin-angiotensin blockade from valsartan.
What to do
Never co-administer. Allow a washout of at least 36 hours when switching between an ACE inhibitor and sacubitril/valsartan in either direction.
What to ask your pharmacist
