Summary

Triptans combined with antipsychotics have a theoretical risk of serotonin syndrome, and altered dopaminergic signalling could in principle favor neuroleptic malignant syndrome. Documented cases with olanzapine are rare.

What this interaction means in plain words

Olanzapine and Sumatriptan intranasal have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Possible additive serotonergic effects, as olanzapine has 5-HT2A antagonism and sumatriptan is a 5-HT1B/1D agonist; the mechanism is largely theoretical.

What to do

Co-administration is generally acceptable; counsel the patient to report agitation, fever, tremor, rigidity or diarrhea, and evaluate promptly for serotonin toxicity if these appear.

What to ask your pharmacist

  • Do I need to tell my doctor at the next visit?
