Summary
Salsalate is a salicylate NSAID that can reduce the blood pressure lowering effect of olmesartan. In older or volume-depleted patients the combination may also worsen renal function and raise potassium.
What this interaction means in plain words
Olmesartan and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and opposes ARB effect, adding risk of acute kidney injury and hyperkalemia.
What to do
Monitor blood pressure, serum creatinine and potassium, and keep the patient well hydrated. Use the lowest effective salsalate dose or choose an alternative analgesic like paracetamol.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
